Select the links below to view resources about using nFORM 2.0 for performance measures data collection and reporting, or to view resources on conducting continuous quality improvement (CQI).

Please contact your grantee organization's site administrator(s) for help with log in issues including locked accounts and resetting passwords. Site administrators can leave a voice mail for Mathematica at 844-619-6841 if they are also unable to log into nFORM 2.0.

If you need additional help after reviewing the resources, you may request technical assistance (TA):

For nFORM 2.0 TA, submit a ticket to the nFORM 2.0 help desk. You must be logged into nFORM 2.0 to submit a ticket on the Help tab.
For CQI TA, email the CQI TA team at hmrfcqi@mathematica-mpr.com.

As a reminder, never include clients' personally identifiable information (PII)—such as client names and phone numbers—when contacting the TA teams. Also, never take screenshots of PII from nFORM 2.0.
